The Fan Yellow Card form is filled out by a club official, online.  The form contents are automatically sent to the Administrative Coordinator.

Any forms with issues e.g. not by a club official, incomplete info on offender etc. are forwarded to DOC for investigation by the Cosmos PAD, DOC & YAD. The fan you report on will not be told specifically who reported.

The fan will be contacted with the appropriate action e.g. suspension from all Cosmos games & practices for a set period, or, longer if the PAD committee finds it necessary.
